Help needed, sigma 500mm f4.5 autofucs issues

I'll begin by apologising if I can't quite explain this properly or even if this is the right place. I couldn't see a help or technical area of the forum.
I've recently exchanged my AF-I 400mm f2.8 in as the 6.5kg was near impossible to use on a daily basis, especially on hikes or walks. I purchased instead a Sigma 500mm f4.5 EX DG HSM APO. It has excellent reviews and is right in the sweet spot for my own needs and desires, fast prime but relatively small and portable, great image quality.

However I'm not sure if it's something I'm doing wrong or this specific lens example, it cannot focus accurately. I have recently tested it on 2 occasions where it seems to be unable to focus, when looking through the viewfinder the target looks perfectly in focus(to me) however when the shot is taken it comes out.. just off. If this is also possibly related it WONT focus in liveview, I tested all my lenses today and they all worked in liveview with the same settings but this lens didn't. It tries then flashes red and stops.

I have tried all the fixes I can Google, including even taking test shots adjusting the af fine tune in between. Nothing seems to fix it, it also "hunts" ever so slightly when using af-c, even on a completely stationary target whilst the lens is also stationary.

Is this something fixable by me, by visiting a specialist, or is it just a return to MPB?
