Sony shooters, does your 1.4tc do this

I ended up using my JIS 00 as my googling said that is the screw type used by Sony. I didn't test the PH one to feel the fit difference.
I found a few screws that could accept just the slightest bit of tightening, very slight. Nothing was really loose. All my TCs were fine. My 600GM and 100-400 were fine. One on my 300GM and two on the A1II mount accepted the small amount of tightening. Considering how new my A1II is I think it goes to show that the camera mount is the most likely one to loosen and takes the most stress compared to the lens/TC mounts.
The two closest to the top on my new A1 were the ones that were slightly loose and that makes sense because that would probably have the most stress in that area. I think from this point forward I’m gonna put a reminder in my phone to just go through and check these things Every 3 to 4 months. It really doesn’t take long at all to do the whole kit just to err on the side of safety. I’m also going to be very aware of how I grab it from this point onward and try to minimize any stress in that area.

Use a tiny bit of blue loctite, which will go a long way and prevent premature loosening of these screws . Just remove one screw at a time, put a tiny amount of loctite on the thread (very little ) and thread it back in snugly, then repeat with next screw.

I have a couple bottles of blue 242 that I use often but it's pretty old. I might get a fresh one
 
I just wonder how many folks are walking around with this same issue and not aware of it? I am extremely observant and did not catch it until it got that bad.
You would be surprised, most have no idea this issue exists until it's too late. It's much more common with heavy tele lenses, because folks feel the camera mount should be strong enough to handle the weight but that is rarely the case. As you experienced, the flange screws will come loose over time.
