Unable to post pictures

The typical way I post pictures is to hit CMD-P (Ctrl-P in Windows) which brings up a dialog box to navigate on my hard drive and select a photo. I don't use the post photo icons (though they still show up on my screen).

And of course photos should be sRGB jpegs less than 1MB in total size and sized to no more than 1200x800 pixels.

One possibility: the Toggle BB Code icon (square brackets) has been clicked.
Another: the Preview icon (to the right of Toggle BB Code icon) has been clicked.
